About us

Objectives

Dans cette pathologie rare, il existe très peu d'étude sur l'intérêt des traitements locaux chirurgicaux ou de radiologie interventionnelle dans la prise en charge de métastases hépatiques. Nous souhaitons apporter notre expérience des traitements locaux dans la littérature : au centre Léon Bérard l'utilisation de technique chirurgicale, et à Nice l’utilisation techniques de radiologie interventionnelle et comparer les deux approches.

 

Objectif principal : 

 

Evaluer l'efficacité du traitement percutanée par thermo-ablation pour les patients oligo-métastatiques au niveau hépatique d'un mélanome uvéal en comparaison de la technique de chirurgie. 

 

Objectif secondaire :

Evaluer la sécurité de la technique percutanée en comparaison de la technique de chirurgie 

Critères d’évaluation :

Critère Principal : Survie globale

Critère Secondaire : survie sans progression, complications grade >III Clavien Dindo, réponse radiologique (critère mRecist), contrôle tumoral local

(a) Critère d'inclusion : tout patient ayant bénéficié d’un traitement par thermodestruction percutané de lésion hépatique secondaire d’un mélanome choroïdien dans les 10 dernières années.

Critère d'exclusion : contre-indication classique à la thermodestruction percutanée soit : patient contre indiqué à l’anesthésie générale, PS> ou égal à 2, ascite, dysfonction hépatique sévère, lésions secondaire extra hépatiques diffuses ou rapidement progressive
