About us

Objectives

Objectif principal : l’implication du médecin traitant en lien avec la diminution des hospitalisations.

Objectifs secondaires : évaluer les besoins en formation complémentaire, évaluer les besoins de contacts nécessaires au médecin traitant pour une orientation spécialisée rapide, évaluer les besoins d’aides nécessaires à une prise en charge ambulatoire optimale, évaluer les motifs d’hospitalisation évitables.
